Return of questionnaire procedure No: 012D

1. Introduction

This SOP offers guidance on the procedure to follow after the return of the GP despatched questionnaire.

2. Responsibilities

Research nurses are responsible for ensuring that the procedure following the return of a questionnaire is correctly followed.

3. Equipment

  • None

4. Method

Ensure to follow the steps laid down by figure 1, following the return of a questionnaire.

Following the return of a questionnaire with a family history suggesting an affected sibling

CALL 1 = Telephone Index case

Complete family screening questionnaire and ask index case to call sibling to confirm interest. Indicate you will call to verify interest in a day or so.

CALL 2 = Rephone Index case to confirm sibling interest

NO
  Exclude family if no interested sibling pair can be made
 
CALL 3 = Telephone Sibling

Complete sibling questionnaire
 
Obtain GP records for Index and sibling

Ensure GP details correspond to subject responses for eligibility Check with GPs that their patient are suitable and well enough to take part
 
CALL(S) 4 = Telephone index and sibling and arrange the visit(s)

1. Obtain consent
2. Personal history questionnaire answered
3. Record phenotypic measurements

 

  • Ensure to complete each section before continuing onto the next.

  • Ensure to maintain subject confidentiality at all times.
